Please note that all new clients are required to complete an Initial Assessment before beginning services with Pliyo.

This allows us to review your medical history and provide the most personalized treatment for your goals.

Where are you located?

Pliyo currently offers in-home, concierge service for Downtown Chicago neighborhoods, including:

Lincoln Park, Lakeview, River North, the Loop, and Roscoe Village.

To ensure we can accommodate your location, please submit a contact form to confirm prior to booking to ensure that you are within our service area.

Do you take Insurance?

No, we are a private pay practice. We do accept HSA/FSA funds.

This allows us to prioritize care without the restrictions, prior authorizations, or visit limits required by insurance companies.

Some clients may be able to submit a Superbill for out-of-network reimbursement. Reimbursement depends on your individual plan.

What is concierge care?

Concierge care means private, one-on-one services delivered in the comfort of your home or residential fitness facility.

It also means direct access and communication with your provider between appointments. Rather than fitting into a traditional clinic model, care is designed around your needs, providing a more personalized, flexible, and supportive experience from start to finish.
